• Graduate Advisory Committee Prelim Evaluation Rubric (to be completed at preliminary oral examination of PhD candidates)
• Graduate Advisory Committee Thesis Defense Evaluation Rubric (to be completed at final examination of all Master's and PhD candidates)
• Graduate Student Exit Questionnaire (to be completed at time of Exit Interview of all Master's and PhD candidates and provided to Department Graduate Program staff)
• Anonymous Graduate Student Exit Survey (to be completed during Exit Interview of all Master's and PhD candidates and provided to department head)
• Core Sub-Discipline Assessment (submitted with Plan of Graduate Work whenever competency in a sub-discipline is justified through prior course work and/or professional experience)
• Exit Seminar Scheduling Form (to be completed by the student and e-mailed to Jot Smyth about 2 weeks prior to the scheduled seminar)
• Graduate Student Curriculum Vitae (a cumulative report submitted by students at final Master's or PhD examination)
• Annual Advisory Committee Meeting Report (a progress report submitted by student advisors once a year)
